Those obsessed fans keep calling him Captain Kirk all the time, rather than just recognizing him as just being a famous actor. Many crazed fans just couldn't separate fiction from reality, so he wrote a book about them.
Bill Shatner found fame and fortune thanks to the Kirk character, so think ridiculing his fans, even the obsessed ones is a bit cheeky. He was responsible for some pretty outragous material himself in his day, all in the "Kirk" style naturally.
